One man dies, two injured in accident while working on water tower

MARSHALL, Minn. - One man died and two were seriously injured Monday when they apparently fell off scaffolding while painting the water tower in Marshall.

All three of the men are from South Dakota and were working for Maguire Iron Inc., of Sioux Falls, S.D.

The names of the men have not been released, but police said the man who died was a 29-year-old from Sisseton. The injured men - a 33-year-old from Sioux Falls and a 30-year-old from Bridgewater - were taken to an unknown hospital, police said.

Man dies in grain elevator fall

Shawnee, KS -- A Shawnee Milling Company employee fell to his death Monday morning while using the man-lift vertical conveyor belt that transports workers to upper floors in the grain elevator.

William "Rick" Engler, 57, was employed at the mill nearly 35 years as an elevator operator. He traveled the man-lift several times each day in his duties for production of mill mixes with wheat and corn.

His fatal fall occurred on the north side of the grain elevator. Officials don't know if Engler slipped, or if he suffered a medical problem.


Police Make Arrests In Massage Parlor Shooting

Police were called to The Swedish Institute in the 7000 block of John Carpenter Freeway at about 6:25 a.m. Monday.

According to police reports, a gunbattle ensued after three people entered the massage parlor in what police believe to be a robbery attempt.

Investigators found a female employee dead from multiple gun shot wounds. The massage parlor's manager also was shot, and is at Parkland Memorial Hospital in stable condition.


Construction worker killed in accident

One worker was killed and another was trapped for 45 minutes Wednesday morning when a concrete wall collapsed on them at a Kendall house under construction.

The two men were part of an eight-person construction team building the home at 9760 SW 60th St., a few blocks from Glades Middle School.

Just before 9 a.m., while concrete was being poured into the walls of a carport-type structure, the building collapsed on the two workers, said Miami-Dade police Detective Randy Rossman, a department spokesman.

The victim, a 49-year-old man, was dead when police and fire rescue arrived at the scene. The man, whose name was withheld pending notification of family, had been working on the roof of the structure.


Construction worker dies on the job

Charles D. Ballard, an employee of Fahs-Rolston Paving Corp. in Binghamton, was struck and killed about 5:30 a.m. on a busy section of Interstate 90 near Exit 5A in Albany, state police at Loudonville said.

Ballard crossed a barrier and stepped into the path of an eastbound tractor-trailer, troopers said. He was taken by ambulance to Albany Medical Center, where he was pronounced dead.

Officials are calling the accident a pedestrian error. Ballard and a co-worker were taking down the floodlights after a night shift when he hopped the concrete barrier and stepped on to the eastbound side into traffic, Peter Van Keuren, a spokesman with the state Department of Transportation's Capitol Region Office, told the Times-Union of Albany.

Indianapolis - August 28 - The Marion County Sheriff's Department calls for an investigation into what happened after industrial accident in Castleton. A man on the job is killed after falling from a grain elevator catwalk.

Employees sit in shock while investigators work to piece together an accident. Working to find out why the Hubbard Feeds worker fell thirty feet to his death. "He was in the process of stringing some air hose lines for some work to be done." "It's an area that's found between the two grain bands. There's an open area that goes from basically the floor to the top with different layers of catwalks that go between the two bins." says Jay Settergeb of the Lawrence Township Fire Department.


Worker dies in 30-foot fall

New York -- A construction worker fell 30 feet to his death and another worker was injured yesterday after both men lost their balance on scaffolding at a Flatbush church, police said.

The unidentified workers were repairing a Plexiglas sheet over a stained-glass window behind Our Lady of Refuge Catholic Church at about 9:30 a.m. They clung to each other before falling, a police spokeswoman said.

Man killed in semi mishap

Ft. Myers, FL - A Sarasota man died Friday in an accident involving a semitrailer and a Mack truck dumping Hurricane Charley debris at the Gulf Coast Landfill off State Road 82, according to the Florida Highway Patrol.

The man, whose name was not released Friday night pending notification of next of kin, was sitting inside his truck shortly before 3:45 p.m. as a device pushed debris out the back and into the landfill. He was parked next to a semitrailer dumping roofing material from Charlotte County.

The back of the trailer was rising so the debris would fall from the back. Material near the front was too heavy, causing the trailer to fall on its side and land on top of the Mack truck. The man was crushed and died instantly, FHP Sgt. Owen Keen said.


Worker killed, three others injured in accident while painting freeway overpass

DETROIT -- A construction platform fell Friday as four workmen were painting a highway overpass, killing one worker and critically injuring the other three, police said.

The men (Jorge Anaya, 44), who worked for a company contracted by the Michigan Department of Transportation, were working on an Interstate 75 overpass in southwest Detroit.
They were attempting to lower a 20-by-30-foot platform when a cable holding it up snapped shortly after 11 a.m., police said.

"For some reason, the platform gave way. All four men fell and hit their heads," police Sgt. John Claybourne said.

The workers were not wearing safety harnesses, said James Tate, another police spokesman.

Wall's tumble sudden

CLAYTON TWP, MI - Percy Frank died in his son's arms Wednesday night after a wall they were working on collapsed in a deluge of dirt and cinder blocks.

Frank, 73, of Flint mainly oversaw his family's Mt. Morris Township cement business but enjoyed work too much to stay away from job sites. "He was there to guide and mentor," said his daughter, Lavern Rogers of Grand Blanc.

Rogers said her father was standing near the door of a Calkins Road home Wednesday night when a wall suddenly gave way, trapping him and another worker beneath the front porch.


UTSA investigating second construction death in month

San Antonio -- Officials at the University of Texas at San Antonio are investigating the second construction-related death in less than a month.

On Thursday afternoon, a man was killed when the front loader he was operating flipped over at a construction site on the campus' south end, UTSA officials said today. He was not wearing a seatbelt and died at the scene. "The operator of that truck was caught under that tractor and he did die," David Gabler, a UTSA spokesperson, said.

Another person, working for a separate company, died on another construction project at UTSA on Aug. 2.


Mecklenburg worker electrocuted at pool

(CHARLOTTE) - State and local officials are trying to determine why a Mecklenburg County employee was electrocuted during routine maintenance work at the Aquatic Center.

Michael McNally apparently touched a live wire while working on a light fixture in the ceiling about 10 a.m. He was pronounced dead soon after at Carolinas Medical Center.


Barrick miner dies in hauling accident

ELKO - Underground miner Ray Grof, 39, was killed Tuesday at Barrick Goldstrike Mines Inc.'s Rodeo Mine north of Carlin. The death happened at about 3 p.m. at Rodeo, according to Barrick Gold Corp. spokesman Rod Jimenez.

According to the MSHA fatality bulletin, the accident is classified as an underground haulage death at Meikle Mine. Rodeo is part of Meikle.

Grof's death is the second hardrock mining fatality in Nevada this year. Dave Miller, 43, was killed June 29 at the Mineral Ridge Mine 50 miles west of Tonopah near Silver Peak when his water truck went out of control.


Window worker falls to death in Houston

HOUSTON -- A man apparently working to replace a fifth-story window at a Houston hotel fell to his death today. Houston police say the man was dead at the scene.


Hamilton man dies in Bordentown factory accident

BORDENTOWN TOWNSHIP, NJ - A Hamilton man died yesterday after a large metal beam fell on him at the transformer factory where he worked, police said.

Carl Powell Jr., 67, of Andrea Lane suffered "massive internal injuries" in the freak accident at the NWL Transformers plant and died a short time later in Capital Health System at Fuld hospital's trauma center in Trenton, police said. More here.


Man Dies When Backhoe Tips Into Water

VIERA, Fla. -- A construction worker is dead after his 40-ton backhoe slid into a manmade lake at a Brevard County golf course.

Officials say the man was driving the track-equipped excavation machine late Tuesday afternoon at the future site of the Duran golf course near Viera. A rain-soaked embankment gave way, toppling the big machine into the water and submerging the cab in water and mud.


Worker killed in Miami construction accident

MIAMI - A 49-year-old construction worker was killed and another injured Wednesday when a wall collapsed at a construction site, fire officials said.

Eight workers were pouring concrete into walls in a house under construction in southwest Miami-Dade County when the outside front wall somehow collapsed, said Miami-Dade County Fire Rescue spokesman Lt. Louie Fernandez.

Fernandez said the workers were trapped by falling debris of concrete and plywood. Rescuers took 40 minutes to remove the injured worker from the debris. The worker was taken to Ryder Trauma Center at Jackson Memorial Hospital in serious condition.

The accident appeared similar to one last month in Martin County where two construction workers were killed when a three-story building collapsed as they also poured concrete into the walls. The concrete company was fined $2.4 million this week for failing to buy workers compensation insurance for its employees.



Freeport man killed in Vandy steel plant accident

VANDERGRIFT, PA — A Freeport steelworker was crushed to death yesterday when a roll weighing as much as 35,000 pounds fell on him at the Vandergrift plant of Allegheny Ludlum Steel, officials said.

Authorities identified the victim as Michael Carney, 50, of Main Street.

The accident occurred about 8:20 a.m. in the plant’s temper mill, Fritz said. Carney was helping to change a rolling mill roll, Fritz said. The rolling mill accepts the slabs of steel arriving from the tunnel furnace, and rolls them to the proper thickness or gauge to meet the customer’s order.

There are two rolls — one on top of another — and the top roll was being replaced yesterday with the assistance of a crane operated by another employee when the accident occurred, Fritz said.

While the roll was being lifted the crane shifted for an unknown reason and dropped the roll on Carney, Fritz said. Vandergrift police officer Kasimer Maszgay said he was told the rolling mill roll might weigh from 25,000 to 35,000 pounds.


Worker electrocuted

UPPER MERION, PA - A 32-year-old business owner and subcontractor died Monday when the boom he was operating at a work site came into contact with live wires, sending a fatal jolt of electricity through his body.

Allen Smucker, the owner of Lancaster Poured Walls Inc., was holding on to a pourer when the boom of the cement truck came into contact with the wires, Upper Merion police Sgt. Jeffrey McCabe said. Smucker's colleague was also reportedly touching the boom, but he was "blown off" and sustained burns to his hands.


Worker killed on paving job

MIDDLETOWN, Conn. -- The Occupational Safety and Health Administration is investigating the death of a Colchester man while working on a paving job. Stephen G. Dowling, 30, died Monday afternoon after he was run over by a Bobcat front-loader driven by a construction worker during a driveway paving job, police said. Dowling was one of several workers from American USA Paving who were redoing a driveway at a home on Randolph Road. The company has offices in Colchester and Middletown. Dowling was sitting or kneeling in the driveway, picking weeds or grass from the cracks when he was hit by the machine. The driver of the Bobcat was backing the machine up the driveway when one of the rear wheels struck Dowling about 2:30 p.m.


Carnival worker was electrocuted

BELMONT, NH - The 18-year-old carnival worker who died while setting up a ride at the Belknap County 4-H Fair Friday was electrocuted, according to the state medical examiner's office.

Stephen J. Miner Jr. of Ware, Mass., was on top of a slide ride called Raiders when a wire in the generator used to power the ride's lighted sign malfunctioned, said Bruce Burroughs, chief of the state Tramway and Amusement Ride Safety Bureau.

Miner was assembling one of the ride's safety gates at the time, and investigators assume he touched some part of the ride and received a shock, Burroughs said. A worn-down wire caused an electric current to travel through the ground cable and into the ride, he said. The state medical examiner classified Miner's death as an accident.


Airman killed in Robins accident

ROBINS AIR FORCE BASE, GA - A senior airman was killed and another injured Monday in a fall from a hydraulic lift in a hangar at Robins Air Force Base.

Capt. Tisha Wright, spokeswoman for the 78th Air Base Wing, said the names of the airmen would be withheld for 24 hours after notification of next of kin under a new military policy designed to give immediate family members time to contact other family members, she said.

Both airmen were members of the 78th Civil Engineer Squadron, she said. The second airman was in serious condition Monday afternoon at The Medical Center of Central Georgia, she said.

The two airmen were on the lift changing light bulbs in a hangar when the accident occurred about 9 a.m., Wright said. She declined to elaborate.


Crash kills one

A Waste Management employee died Monday morning after he was struck by a passing vehicle on Carlisle Road in Dover Township.

Michael J. Eaton Sr., 49, was pronounced dead at 10:20 a.m. at York Hospital. An autopsy to determine the cause of death is set for today, the York County Coroner's Office reported .

Eaton, of the 300 block of Myers Road, Chanceford Township, was helping another employee load trash into a truck around 7:30 a.m. along the 4900 block of Carlisle Road when he was hit by a 1985 Buick.


Truck runs over worker, killing him

A worker at a railroad construction site apparently did not hear a slow-moving dump truck's back-up warning signal and the truck ran over him, killing him instantly, according to the Palm Springs Police Department.

The accident was reported just before 10 a.m. Sunday along a railroad access road near Tipton and Wendy roads at the west edge of the city off Highway 111. The name of the 31-year-old worker from Odessa, Texas, was withheld pending notification of his family.

The construction involves new railroad track installation and repair. The worker may not have heard the dump truck because of construction noise and windy conditions at the time, police said.


Joliet officer killed in hit-run

Joliet Police Officer Jonathan E. Walsh recently told his girlfriend not to fret if worse came to worst at his job.

"He just told me a couple of weeks ago that if anything happened to him, he had a good life," Tristen Hnetkovsky said Saturday. "He loved his family. Even though it took his life, he loved his job. He never would have given it up for anything."

Hnetkovsky, 22, spoke as she was comforted by friends at the Joliet townhouse she shared with Walsh, a 3 1/2-year Joliet police veteran who died the night before in a hit-and-run crash.

Walsh was heading toward a Park District facility where a false burglar alarm had sounded. Police said a man whose license had been revoked for drunken driving pulled his sport-utility vehicle in front of Walsh's squad car. After hitting the SUV, Walsh's car careered into a crane parked on the side of the road, causing injuries that led to Walsh's death shortly after his arrival at Silver Cross Hospital.


Man dies in EnCana drilling rig accident

Glenwood Springs, CO -- A 62-year-old Grand Junction man died in a drilling rig accident on an EnCana well Friday afternoon south of Rifle - two weeks short of his planned retirement.

The accident is the second fatality on an EnCana well in western Colorado in the past two months.

The Garfield County Sheriff's Office responded to the scene shortly after 3 p.m. and determined that the incident was an industrial accident and no foul play was involved, Garfield Sheriff Lou Vallario said.

Randall E. Taylor was working on the Wolverine Drilling Rig No. 33 south and east of Rifle when the accident happened. Taylor was up on a platform when the rigging above him broke loose and fell, killing him with blunt force trauma to the head and chest, according to the Garfield County Coroner's Office.


Officer Killed, Partner Hurt Executing Warrant

Florida -- A sheriff's deputy was shot to death and another was wounded as they executed a search warrant in a child pornography case, officials said.

Det. Todd Fatta, 33, was killed when shots hit him in the chest, penetrating his protective vest, Broward County Sheriff Ken Jenne said. Fatta's partner Sgt. Angelo Cedeno was shot in the hand and shoulder.

The occupant of the house, Kenneth Wilk, 42, was taken into custody. Other officers returned fire at Wilk, but he was not injured, Jenne said.


Worker Killed In Fall From Water Tower

Colfax, IA - A Colfax man is dead after an accident at the town's water tower Thursday aftrernoon.

Crews on the scene say they were painting the tower during a routine maintenance check, when the man fell to the ground. Fellow workers say they don't know how it happened. Colfax Police estimate the man fell between 90 and 100 feet. The man's name has not been released, pending notification of relatives. OSHA officials are investigating.


Indiana police officer killed, four wounded by murder suspect

INDIANAPOLIS — A man suspected of killing his mother walked through a city neighborhood early yesterday, shooting at "anything and everything" with a high-powered rifle, killing one police officer and wounding four others, authorities said.

The gunman — identified by police as 33-year-old Kenneth Anderson — was killed in an exchange of gunfire with one of the wounded officers about a block from his mother's house on Indianapolis' south side, police Lt. Paul Ciesielski said.

Officers went to the house about 2 a.m. after receiving reports that a man was shooting and had killed his 66-year-old mother, Ciesielski said.

The first-arriving officer was shot and wounded by Anderson before the officer even had a chance to leave his squad car. Many other officers then responded to the scene.


Worker dies after fall down elevator shaft

MISSOULA, MT - A construction worker died Wednesday after falling 40 feet down an elevator shaft and striking concrete. The man landed on his head, said Battalion Chief Todd Scott of the Missoula Fire Department. The fall happened at about 10 a.m. while the man was working on construction of Gold's Gym, a fitness center. Scott said the man died soon afterward. His name was not released.


City worker killed

Salina, KS -- A city worker was killed Wednesday when he was struck by a van on South Broadway Boulevard. Salina police Lt. Russ Lamer said Shannon Helms, 39, was working on a section of the street when a southbound van struck and killed him and then slammed into the back of a city dump truck. The crash occurred about 1 p.m. in the southbound lanes of Broadway, south of Republic Avenue.


Rhinelander man dies when truck collides with pickup on U.S. 41

Green Bay, WI -- A 49-year-old construction worker died Wednesday when a truck hauling water pipe forced his pickup into a concrete guardrail on U.S. 41 in Ashwaubenon.

Steven Dietz of Rhinelander was northbound in the right lane of U.S. 41 at about 11:15 a.m. when a northbound flatbed moved from the left lane into the right lane, colliding with Dietz’s Silverado pickup just south of the Oneida Street/Waube Lane exit.

Brown County Sheriff’s Department Capt. Randy Schultz said Dietz’s pickup was crushed into the guardrail of the Parkview Road overpass and flipped upside down when it reached the end of the barrier, coming to rest on its top, teetering on the edge of the overpass embankment.


E-N worker killed in crash

San Antonio -- An independent contractor for the San Antonio Express-News circulation department was killed early Wednesday when his truck crashed in Schulenburg.

Thomas Wright, 68, had dropped off bundles of papers in Schulenburg and was heading toward Flatonia on U.S. 90, Schulenburg Police Chief Randy Mican said, when his 2003 Ford F-150 pickup veered off the left side of the highway at about 2:50 a.m.

The truck hit a culvert, went airborne and landed on its side, Mican said


Randolph man killed in crane accident

BERLIN, NH — Terry Corrigan was the sort of fellow who would do anything for anyone, and yesterday many in the Androscoggin Valley were mourning his death.

Corrigan, 47, was killed Tuesday night when a crane he was trying to load onto a trailer overturned. The crane rolled over a guardrail and into a ditch off Frog Pond Road, pinning Corrigan in the cab.

Because of the position of the huge piece of equipment lying on an embankment, rescuers were unable to reach him immediately. A local crane service responded and lifted the crane, allowing firefighters access to remove him from the cab.


Man Dies In Forklift Accident

VERNON, CT -- A local man died from injuries he suffered in an industrial accident Wednesday, police said. Dave Turner, 46, a longtime employee of Anocoil Corp., became pinned between a forklift and a compactor outside the building at 60 E. Main St. in the Rockville section of town.Employees discovered him about 1:10 p.m., police said, and had removed him by the time officers arrived.

Vernon police Sgt. Paul Miffitt said it appears Turner had been operating the forklift, which was carrying a trash bin. He got out, possibly to move something, and the forklift rolled into him, pinning him against a large compactor behind the building, Miffitt said.


Former Natick resident dies from tractor injuries

David Prior, 35, a Milford resident and Natick native, died at Boston Medical Center early Tuesday morning from injuries suffered when he was run over by his own tractor at Newton's Woodland Country Club, a hospital spokesman said.

The groundskeeper had been in critical condition in the intensive care unit since being flown to the Boston hospital by medical helicopter last Thursday.


Construction worker dies in Hollywood elevator shaft

HOLLYWOOD, Fla. - A construction worker died when he fell through an uncovered elevator shaft on the 19th floor of a beachfront high-rise, officials said.

Jose Altamirano, 42, of Miami, was seeking shelter from a morning downpour Tuesday when he ran to get behind a steel panel, then slipped and fell on a plywood deck near the elevator shaft, said Daniel Whiteman, president of Coastal Construction Services LLC, the project's contractor.
Altamirano's body was found near the elevator pit, said Matt Phillips, a spokesman for the Hollywood Fire-Rescue Department.

It wasn't clear how he slipped through the elevator, which was secured with a guardrail.


Construction worker killed in fatal house fire

Police and firefighters are investigating a fatal house fire in which a construction worker was killed Wednesday afternoon in Chevy Chase Village.

After extinguishing the fire at 31 W. Irving St., rescue workers found the body of Mauricio Melvin Ortiz, 35, of Lanham, in the basement and he was pronounced dead on the scene, said Pete Piringer, Montgomery County Fire and Rescue Services spokesman.

Ortiz and another man, both construction workers from Kenwood Construction Inc. of Gaithersburg, were performing renovations to the basement floor of the two-and-a-half story home at about 3:45 p.m. when a flash fire occurred, enveloping Ortiz who was unable to escape the house, Piringer said. The second worker evacuated the house without injuries, Piringer said.


Man killed in northeast Ohio gas well accident

NEWBURY, Ohio -- A contractor performing maintenance on a natural gas well was killed when excess pressure thrust part of a metal pipe into the air, striking his head, police say.

Joe Wilson
, 49, of Atwater, was pronounced dead at the scene of the accident Tuesday, said Geagua County Sheriff Daniel McClelland.

Wilson was one of three men repairing a leak in the well's holding tank for W. Pol Contracting Co. of Ravenna. The other workers were not injured.

Newbury Township Police Chief Santo Battaglia said Wilson was hit in the head by a 2-inch piece of pipe that broke off in the blast.

"The extent of his injuries were astronomical," Battaglia said.


Marshall miner killed at McElroy mine

MOUNDSVILLE, W.Va. - A coal miner was killed early Tuesday when coal and rock fell off the side or "rib" of the underground mine and hit him, a company spokesman said.

Joseph Sabo, 57, of Glen Easton was struck at about 4:30 a.m. at Consol Energy's McElroy Coal Co. mine near Moundsville. He was taken to Reynolds Memorial Hospital in Glen Dale, where he was pronounced dead, said Thomas Hoffman, a Consol spokesman in Pittsburgh.

Sabo was a roof bolter who was working in a continuous mining section, Hoffman said.


Power line kills Pepco worker at College Park

A Potomac Electric Power Co. worker died yesterday morning after apparently touching a charged line at the utility company's College Park substation, officials said.

A company spokeswoman identified the employee as Michael Ashurst, 47.

Worker dies after being buried by corn

WACO, Texas -- A 47-year-old worker at a Waco grain elevator is dead after he was buried in corn while trying to retrieve a tool.

Investigators say Stanley Rollins fell into a grain silo at Clement Grain Company yesterday while trying to retrieve a wrench he dropped.

The elevator hadn't been turned off, and Rollins was buried by arriving grain. Two co-workers tried to pull him out, but the force of the falling corn was too strong.

Crews recovered his body about an hour later. An autopsy has been ordered.


Contract worker dies at Pearl Harbor

PEARL HARBOR, Hawaii — A civilian contract worker died Monday after being found unconscious at Pearl Harbor, the Navy said.

The man apparently died of a heart attack, according to Lt. Cmdr. Jeff Davis.

The man was discovered in the morning by a fellow worker at the naval station’s Bravo piers, Davis said.

He was pronounced dead at Kapiolani Medical Center at Pali Momi.

The worker was employed by HSI-Mechanical Inc., a subcontractor of Triton Marine Construction, Davis said. The Navy hired Triton to replace fuel lines under the piers.


Police officer killed in one-car crash

HONOLULU (AP) _ A Honolulu police officer has been killed in a one-car crash on the H-Two Freeway.

Police say the 58-year-old officer was responding to a call when his car veered onto the shoulder of the freeway and overturned several times. Police say speed may have been a factor in the crash.


Sheriff’s officer killed returning from PGA

SHEBOYGAN, WI — A sheriff’s officer on the way home after providing security at the PGA Championship at Whistling Straits died when his motorcycle was hit from behind, authorities said Monday.

Lt. LeRoy Nennig Jr., 53, a member of the Sheboygan County Sheriff’s Department since 1974, was traveling on State 23 from Haven to his home in Plymouth when the crash happened about 10:20 p.m. Sunday, sheriff’s officials said.


Construction worker killed in accident

A 35-year-old construction worker was killed in a freak accident Friday while working in a manhole.

The man, who was employed by KD Robinson Construction, was working on a pressurized irrigation system for a home in the area of Harbor Park Drive near the Saratoga Springs Marina in Saratoga Springs when the accident happened, said Dennis Harris of the Utah County Sheriff's Office in a statement.

The man drove about a block away and entered a manhole to open up the water line. As he was exiting the manhole with a long, metal water key, a vehicle struck the key, knocking the victim back to the bottom of manhole, Harris said.


Worker's death caused by electrocution

CLEMSON, S.C. - A coroner's report says a worker died from electrocution at a WestPoint Stevens' Clemson Finishing plant earlier this week.

Milford Eugene Carter II, 43, was found in an elevated lift bucket by plant workers on Tuesday.

Carter was an electrician working at the WestPoint Stevens.

Oconee County Coroner Karl Addis said Thursday that he ruled the death an accident after a report showed Carter was electrocuted.

WestPoint Stevens is cooperating with a federal Occupational Safety and Health Administration investigation and will review its safety procedures before similar work is resumed, company spokeswoman Lorraine Miller said.


Well explosion death toll now 3

A Perry County gas-and-oil well that exploded Wednesday into a 100-foot torch continued to burn yesterday, as a third worker died of his injuries.

William Douglas Bell, 37, of Helenwood, Tenn., died yesterday at the University of Cincinnati Hospital.

Two co-workers, Bill Chandler, 36, of Lost Creek in Perry County, and Patrick Jeffers, 29, of Winfield, Tenn., died Wednesday.

The 20-year-old oil-and-gas well on a hillside above Ky. 550 in Bulan, a small community outside of Hazard, exploded about 2:15 p.m. Wednesday.

Bad weather yesterday evening forced crews to suspend firefighting efforts until today, Kentucky State Police said.

Investigators said they did not know what caused the explosion.

"I'd be afraid to speculate," said Rick Bender, director of the oil and gas division of the Kentucky Cabinet for Environmental and Public Protection.

Bender said the well, which produced about 10 barrels of oil a day, is owned by Journey Operating LCC of Houston, which had hired a Tennessee company, Myers Well Service, to clean the "down-hole pump" on the rig.


Guyanese dies as elevator malfunctions in Times Sq. tower

An elevator operator in one of Times Square's newest towers was killed on Thursday when a freight elevator malfunctioned and propelled his cab up into a rooftop bulkhead above the 37th floor.

The accident occurred shortly before 5 p.m. in 5 Times Square, a sleek, three-year-old glass office tower on Seventh Avenue between 41st and 42nd Streets. The building is the world headquarters for Ernst & Young, the accounting firm.

The man who was killed, Carl E. DeClercq, a 63-year-old immigrant from Guyana, was alone in the cab, the authorities said. According to fellow workers, he was descending in his elevator to leave the building at the end of his shift.
